Admissions
If you or someone you know believes a child or adolescent youth could benefit from Positive Changes Partial Hospitalization Program, contact our admissions department. Our intake coordinator will complete a free and confidential risk assessment with you over the phone to determine if the youth meets criteria for services at Positive Changes. If the youth meets criteria and you are interested in admitting to our program, our intake coordinator will set up an assessment appointment which will take approximately two hours. During the assessment appointment, you will meet at the Positive Changes location the youth will attend with an intake therapist, a member of our nursing staff and our milieu director to discuss transportation to and from program.

- All medications your child is currently prescribed
- A copy of your child’s birth certificate
- Current shot records
- Insurance or Medicaid ID Cards
- Your child’s original social security card
- Any past psychological testing/evaluations *if applicable*
- A custody decree or guardianship paperwork *if applicable*
- A current Individual Education Plan (IEP) or 504 plan *if applicable*
- A copy of discharge paperwork from inpatient facility *if applicable”
- Your child must be present during the assessment
